 What is the Medical Services Card?

The Medical Services Card is a permanent, plastic identification card issued to clients who receive medical assistance. Having a Medical Services Card does not mean that the client is currently eligible.

Medical providers need to confirm current eligibility at the time service is provided. The Medical Services Card replaces the medical identification cards (MAIDs) used to identify medical eligibility.

 What information displays on the Medical Services Card?

The following information displays on the Medical Services Card:

  • The client's full name
  • Date issued, and
  • The ProviderOne Client ID.

The Medical Services Card does not display the program(s) the client is eligible for, coverage dates, or managed care plans.

 How is the Medical Services Card issued?

Medical Services Cards are centrally issued by Health and Recovery Services Administration (HRSA).

 Can a Medical Services Card be issued locally to a client?

 Medical Services Cards can only be mailed to clients from a central location.

 If a client has never had a Medical Services Card, one is mailed to the client once they have been determined eligible for medical assistance.

 How long does it take to receive the Medical Services Card?

Clients can expect to receive their Medical Services Card in 7-10 days from the date they have been approved for medical assistance.

 Can a client receive medical services the same day eligibility is approved?

Once a client has been approved for medical assistance their eligibility information is sent to Health and Recovery Services Administration (HRSA’s) ProviderOne system in real time.

Clients can receive needed medical services on the same day their medical application is approved by giving their full name, Social Security Number (SSN), and/or date of birth to their medical provider who can then can confirm the client's eligibility status through the ProviderOne system.

 Are clients required to have a Medical Services Card in order to receive medical services?

A Medical Services Card is not required to receive medical services. Medical providers can verify the client's eligibility using the client's ProviderOne Client ID, full name, date of birth, and/or Social Security Number.

 Do clients receive new Medical Services Cards each month?

Medical Services Cards are issued only once to a client.

 How can a client replace their Medical Services Card?

Clients may request a replacement Services Card by calling the ProviderOne Help Desk at 1-800-562-3022 or 1-800-848-5429 (TTY/TDD).
