Medicaid eligibility is your pathway to services.
If you receive services through the Developmental Disabilities Community Services, you must have Medicaid coverage. This is because our services are funded through Medicaid programs. Medicaid is also known as Apple Health.
How it Works
- Your DDCS case manager will complete an assessment to determine functional eligibility.
- Either DSHS or the Health Care Authority determines your financial eligibility, depending on where you apply (see STEP TWO below). You will need to provide financial information to complete the application.
- If you are not currently deemed disabled by Social Security, you will need to complete a disability determination with our DDCS financial department.
Steps to Financial Eligibility
STEP ONE: Determine your Long-Term Care Coverage type.
Your case manager will help determine which Medicaid coverage you need, based on your assessment. The following types of Medicaid programs provide long-term care services:
- Home and Community Based Services Waivers
- Roads to Community Living
- Skilled Nursing Facilities and Residential Habilitation Center
- Community First Choice
STEP TWO: Determine where to apply for Medicaid.
Use the guide below to take your next step:
- Apply to the HCA Healthplanfinder (STEP THREE) if you are determined eligible for CFC and are:
- Under age 65.
- Not receiving SSI.
- Not on Medicare.
- Apply to DDCS Long-Term Care Medicaid (STEP FOUR) if you are determined eligible for a waiver program or any long-term care Medicaid service and are:
- Age 65 or older;
- Blind or disabled (any age); or
- Eligible for or receive SSI.
STEP THREE: Apply for Apple Health through the Health Plan Finder.

STEP FOUR: Apply through DDCS for Long-Term Care Medicaid.
