Finding an Assisted Living Facility Offering This Program
If there is an Assisted Living Facility in your area and you think the person would be eligible, your next step depends on whether the person already receives Medicaid to help pay for long-term care.
If the person already receives Medicaid
Call the person with dementia’s case manager for more information about this program. The person’s DSHS care plan has the case manager’s contact information. If you can’t find the number, contact your local Home & Community Services office.
The case manager can also help identify other alternatives if no Assisted Living Facilities offer this program in your community or space is not currently available in ones that do.
If the person has not applied for Medicaid
The first step is to see if the person with dementia is eligible to receive Medicaid. Learn more about applying for Medicaid.
