Person-Centered Planning is a process to discover what is important to you and take action to help you live the life you want. It is about listening, learning and building community in support of your goals. That support can come from services, friends, family and other people you want to be part of your life.
We use Person-Centered to describe the service planning you do with your DDA case manager during your annual assessment. We also use it to describe a service that’s guided by a professional Person-Centered Plan Facilitator.
Facilitated Person-Centered Planning
With the help of a professional facilitator, this service brings together people in your life to create a vision for your future, with action steps to reach your goals. It’s a positive, community-building experience that creates space to dream and think of ways to turn possibilities into reality.
Facilitated PCP is especially helpful in planning ahead for big transitions in your life, like graduating high school, moving out of your family home or working toward career goals. It’s also helpful for everyday goals and connection building.

Person-Centered Service Planning
At least once a year, your DDA case manager will reach out to schedule a meeting with you to talk about the kinds of support you need to live the life you want. You can invite anyone you choose to come to this meeting. Your DDA case manager will work with you to create a Person-Centered Service Plan that includes the type of services you need, how much of it you need and who you choose to provide it.
It also includes other kinds of support in your life, some of which might come from resources in your community, friends, family and others in your life. Because it is focused on you, the plan reflects your strengths, choices and needs. To help you make the most of your meeting, your DDA case manager will send you a My Page Profile to complete ahead of time. You can fill it out on your own or with someone you choose.
