Specialized Habilitation
This home and community-based waiver service helps individuals handle complicated situations and changes, problem-solve when things don’t go well, build and maintain healthy relationships, learn safety skills, self-advocacy and more. It’s a goal-based service with action steps based on the person’s core values. This service may be provided more frequently, up to 90 days, for those with an urgent need for Specialized Habilitation. Stabilization is intended to help an individual return to a more typical level of support provided by standalone Specialized Habilitation.
Staff and Family Consultation
This home and community-based waiver service helps an individual’s family and support staff understand how to better support their needs, learn techniques on how to use new equipment or technology for safety, learn parenting skills, get on the same page with each other and more. This service may be provided more frequently, up to 90 days, for those with an urgent need for Staff and Family Consultation. Stabilization is intended to help an individual return to a more typical level of support provided by standalone Staff and Family Consultation.
Mobile Diversion
Mobile diversion is a type of diversion service (see below) that’s intended to prevent the loss of a person’s home, their caregiver(s), their employment or access to other community services. These services are provided in the person’s own home and typical community settings. By bringing supports to the person where they live, mobile diversion seeks to minimize disruption to their life while giving them the support they need to continue living at home. Please Note: Mobile diversion services are available in limited areas at this time. This service is managed by the Regional Clinical Team in each DDA region.
What if I need to live out of my home for a little bit to keep from going into the hospital or an institution?
Community-Based Diversion Services
These are temporary, out-of-home services to help people with behavioral health needs that cannot safely be met at home. Diversion services help prevent hospitalization or institutionalization.
Diversion Beds
People who access diversion bed services will develop skills and connect with resources in order to transition to a community setting of their choice. Diversion services provide 24-hour, one on one supports. Diversion bed facilities are located across the state. Referrals are managed by the Regional Clinical Team in each DDA region.
State-Operated Stabilization, Assessment and Intervention Facility
People who meet eligibility for the Stabilization, Assessment and Intervention Facility will develop skills based on up to three personalized community-based goals to transition (or return) to the community of their choice. SAIF is a 90-day service that provides 24-hour support for up to six adult clients. The facility is located in Pierce County. DDA Policy 4.25 SAIF Fact Sheet
Yakima Valley School Crisis Stabilization
This service is for adults at risk of hospitalization without medical need, or when they remain hospitalized without medical need. Individuals aged 16-20 may be admitted only if there are no service options available in the community to appropriately meet their needs. The primary purpose of the service is for the client to stabilize and transition to supports in their community of choice. This differs from intermediate care facility services that provide continuous active treatment. The crisis stabilization services offer 24-hour support in a cottage on the campus of Yakima Valley School, a residential habilitation center certified as a nursing facility in Selah.
