Community First Choice (All Ages)
CFC offers a variety of services to help you be more independent:
- Personal Care
- Relief Care
- Assistive Technology
- Personal Emergency Response System
- Caregiver Management Training
- Community Transition Services
- Nurse Delegation
- Skills Acquisition Training
CFC services are provided in your own home by a person or an agency you choose. Adult family homes and assisted living facilities also provide CFC.
CFC is available through both DDCS and HCS. Children who do not qualify for DDCS may be eligible for CFC through our Specialized Unit.
