If you are an Authorized Representative and want to follow up on claim status:
- Multiple claim status requests can be sent by fax to the local Disability Determination Services office closest can request multiple claim status updates via fax by sending it to the local DDS office
- The DDDS will respond through fax indicating status updates to Authorized Representative and will be noted as claim is in “pending” or “closed” status.
- If specific claim status is being requested such as Consultative Examination and medical records request, please contact the DDDS Customer Service Line at 1-800-562-6074
